Registration
Kids and Cultures Family Camp is for children who have completed grades 1 to 7. Children must be accompanied by an adult. To reserve your family's spot, please complete this registration form and send a $50 non-refundable deposit within two weeks of registering (or you may send the total amount due).
Parent/Guardian Permission and Release statement
By registering, I hereby give my child(ren) permission to attend Kids and Cultures Family Camp between the hours of 9:00 and 12 noon on August 10 to 12, 2009. I or another responsible adult will attend and participate with my child(ren).
I hereby release Kids and Cultures and the host location from all legal liability for my child(ren).
I authorize the staff/volunteers of Kids and Cultures to take and use photographs and audio/video footage of the members of my family as needed for their records and/or publicity programs.
